Support coordination
Support coordination helps you to fully utilise your NDIS plan by ensuring you understand what funding you have and how to use it, connecting you to services and increasing your confidence in navigating the NDIS. Support coordination can include the following:

Navigating the NDIS - Helping you understand what is included in your NDIS plan and supporting you to prepare for NDIS review meetings.

Connection and Coordination- Supporting you to access NDIS, government and community services that you need. Coordinate meetings with your services and ensuring your providers are working collaboratively.

Support- Supporting you to self-advocate by building your confidence and skills and assisting you to manage unexpected situations or crisis by ensuring you have access to the supports that you need. Increasing participants’ social and economic independence and participation through the provision of capacity building supports.

Activ8 Mind’s support coordinators work with NDIS participants aged 10 and older and can provide support for individuals living with physical, psychosocial, neurological, sensory, cognitive and/or intellectual disability diagnoses.
Your support coordinator will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in your community.
Our support coordinators are compassionate, experienced and focused on supporting you every step of the way through your NDIS journey.
Specialist Support Coordination - NOW AVAILABLE
Activ8 Mind’s specialist support coordinators work with NDIs participants aged 10 and older who have high and complex needs requiring a higher level of support. They can provide support for individuals living with physical, psychosocial, neurological, sensory, cognitive and/or intellectual disability diagnoses.
Our specialist support coordinators will work with the participant, their families and carers to identify a broad network of supports that can help the participant use their plan effectively and cohesively.
Our specialist support coordinators have extensive knowledge of the NDIS and disability sector and are able to provide compassionate and effective support to address any barriers you have to accessing to the supports you need.

What can I expect from Support Coordination or Specialist Support Coordination services?
A support coordinator works with you to help you understand what is included in your NDIS plan, what services you need to help you meet your needs and achieve your goals, helps connect you to these services, monitors the quality of the services, helps you track your funding and helps you prepare for NDIS plan reviews. A support coordinator can also support you to navigate community and non-NDIS services that you may also need.
A specialist support coordinator provides these services in addition to supporting you to address complex factors by developing a plan to ensure all services are working cohesively with you and supporting you to access any other services you require from NDIS or non-NDIS providers.
What can I expect from Psychosocial Recovery Coaching?
A psychosocial recovery coach is a qualified and experienced coach who understands disability and mental health. They provide you with support in understanding and utilising your NDIS plan, finding appropriate services to meet your needs, managing your budget and preparing for NDIS plan reviews. In addition to these services your psychosocial recovery coach will support you to set short-term recovery goals related to your mental health and develop strategies to achieve these goals.
What is the difference between a Support Coordinator and a Specialist Support Coordinator?
Specialist support coordination is provided for NDIS participants with high and complex needs. A specialist support coordinator supports you to overcome significant barriers that have prevented you from accessing the supports and services you need. They have extensive experience in the disability sector and must be degree qualified to ensure they have the necessary skills to provide this higher level of support.
What is the difference between a Support Coordinator and a Psychosocial Recovery Coach?
Support coordinators have broad experience in the disability sector and knowledge of navigating the NDIS. They are experienced in working with individuals with various disability diagnoses and are able to support participants with various circumstances. Comparatively, a psychosocial recovery coach has specialised knowledge of mental health conditions and/or lived experience with a mental health condition. This provides them with a unique perspective into recovery from a psychosocial disability, as well as the navigation of NDIS and non-NDIS mental health supports.

Can a Support Coordinator or Psychosocial Recovery Coach provide advocacy services?
A support coordinator or psychosocial recovery coach can help you to develop self-advocacy skills to increase your confidence in navigating the NDIS and advocating for your needs. However, support coordinators and psychosocial recovery coaches are not able to provide a direct advocacy role. If you require an advocate we can help you find an advocacy service.

Can I access telehealth Support Coordination or Psychosocial Recovery Coaching?
Absolutely, our support coordinators and psychosocial recovery coaches are able to offer video telehealth sessions and phone consults. Please note that initial meetings will typically be face to face except in exceptional circumstances.

How long will each session be?
Initial appointments are between 1-2 hours. Subsequent appointments will vary in length depending on what the goal of the appointment is.

How often will we meet?
Support coordination and psychosocial recovery coaching services are provided on an ‘as needed’ basis. Your support coordinator or psychosocial recovery coach will be in touch with you at least once per month, either by phone/email or face to face appointments. You can request more or less frequent contact depending on your preferences.

What is the cancellation policy?
​
Activ8 Mind requests that you give 24 hours’ notice if you need to cancel or reschedule your appointment. Sessions cancelled within 24 hours of your scheduled appointment time will incur a cancellation fee except in exceptional circumstances.

How do I make a booking?
​
If you contact us and leave a message we aim to get back to you either that day or the next business day. If you talk to reception, they can help you with any questions and make an appointment for a time that suits you.
